Five years after the Pfizer-BioNTech COVID-19 vaccine was rushed onto the market under emergency authorization, a new analysis by researcher Yaakov Ophir, Ph.D., has concluded that the pivotal clinical trial never demonstrated the vaccine's benefits outweighed its risks. The finding arrives as Health and Human Services Secretary Robert F. Kennedy Jr. moves to terminate the emergency use declarations that governed the vaccine's rollout. Ophir's analysis, published by the Brownstone Institute, examined the same evidence the U.S. Food and Drug Administration reviewed in December 2020 and found a far less favorable picture than the widely publicized 95% efficacy figure suggested.
Ophir, who heads the Mental Health Innovation and Ethics Lab at Ariel University in Israel, determined that depending on the counting period, the vaccine group experienced between two and eight fewer cases of severe COVID-19 compared with the placebo group. Meanwhile, safety data showed numerical excesses of four to 101 participants across clinically consequential adverse-event categories.
"The evidence did not establish that the vaccine's clinically consequential benefits outweighed its potential harms," Ophir wrote. "Nor did the trial establish that vaccination reduced the overall burden of COVID-19-like symptoms, let alone that it reduced person-to-person transmission."
What the 95% efficacy figure actually measured
That headline number only ever covered one narrow slice of the picture: symptomatic COVID-19 confirmed by lab testing, tracked for a little over six weeks. The trial was never designed to show whether the shot stopped people from catching the virus without symptoms or from spreading it to others — and researchers could only confirm testing results for a sliver of participants, less than 9%.
That distinction mattered far more than regulators let on. Officials and employers leaned on vaccination status to justify mandates and vaccine passports, treating a shot with no proven transmission benefit as if it were a shield for the people around the vaccinated person, yet those claims had no foundation in the trial's central finding.
"We should not have been so surprised, then, when it became clear after the EUA that vaccinated people continued to become infected in large numbers," Ophir wrote.
The severe outcomes the trial never proved
Nobody in either the vaccine or placebo group died of COVID-19 during the trial. Researchers pointed to nine severe cases among placebo recipients versus just one among the vaccinated as early proof the shot worked. But a good portion of that gap came from infections recorded before participants were even considered fully protected — cases too early to be reasonably attributed to vaccine-induced immunity.
Narrow that window to the actual pre-specified comparison period, and the gap nearly disappears: one severe case in the vaccine group versus three in the placebo group, out of more than 43,000 people enrolled. This difference is too small to be statistically meaningful.
The safety numbers, meanwhile, ran the other way. The vaccine group included four more participants with serious adverse events judged related to the intervention, seven more with related events leading to withdrawal, and 101 more with severe adverse events.
The comparison regulators never bothered to make
Ophir's central argument is that the original trial report never presented a side-by-side risk-benefit comparison. While some may argue a larger trial would have detected benefits this one missed, Ophir countered that the same logic must apply to potential harms.
"If uncertainty on the efficacy side is counted as a potential benefit, unresolved numerical excesses on the safety side must also be counted as potential risks," he wrote.
None of this required hindsight. The numbers were sitting in the trial publication, its appendix, and the FDA's own review files the whole time; they were just never added up in one place before Ophir did it.
That's the deeper problem this analysis exposes. A novel product was pushed through in months, then mandated for millions of Americans who faced losing their jobs, their access to public life, or their military careers if they refused — all while the government's own trial data left the actual risk-benefit math undone. People deserved that math, and the full picture of potential side effects, before being pressured or coerced into rolling up their sleeves. Full, honest disclosure — not a rushed sales pitch — should be the baseline for any medical product, especially one built on a profit model as lucrative as this one.
"More than five years later, revisiting this evidence is essential if we are to avoid repeating the same mistakes when the next public-health emergency arrives," Ophir concluded. "An emergency may justify speed and action under uncertainty, but it cannot justify counting uncertain benefits generously while discounting uncertain harms."
