Comfortably Numb: How Psychiatry Is Medicating a Nation

Since even one previous attempt multiplies suicide risk by 38 to 40 times, and suicide is the fourth leading cause of death for adults, a proven way to prevent repeat attempts has important public health implications," said the Director of the National Institute of Mental Health, Thomas Insel.18 CBT has some highly practical applications. Researchers in the physiotherapy department at King's College, London, administered a cognitive-behavioral pain management program for patients experiencing chronic pain after cancer treatments such as surgery, radiotherapy, and chemotherapy.
There are a lot of data showing that people who sleep poorly are more likely to relapse [with depression] and that suicide risk is higher," Sussman said. Sleep problems also often require additional medication: 22 percent to 34 percent of patients taking SSRIs are also prescribed sedatives or hypnotics. There can also be bizarre sexual side effects—such as "yawning-excitement syndrome," in which patients experience sexual arousal when they yawn, even progressing to orgasm. "This is probably underreported," Dr. Sussman said.

Internal company documents that became public included a memo to company sales reps that instructed them specifically not to discuss the potential suicide risk with physicians. Other company research suggested that as many as one in four patients on Paxil suffer withdrawal symptoms if they abruptly stop taking the drug. As David Healy, a British psychiatrist who helped spark recent investigations into the potential dangers of SSRIs, puts it, "If you've got a very severe problem, and I treat you with a drug like Paxil, I may save your life, I may save your marriage, I may save your career.

Though much of the previous research had focused on users' suicidal thoughts and actions, this study "focused on things that are at the far more severe end of the spectrum— kids coming into the emergency room following suicide attempts and those who actually die," says Dr.

Enormous evidence linking mind-altering drugs with violent acts In 2005, I reported on this site that Eli Lilly had full knowledge of a 1200% increase in suicide risk for takers of their Prozac drug, a popular anti-depressant SSRI medication. (See http://www.newstarget.com/003086.html ) In 2006, we reported the results of a study published in the Archives of General Psychiatry showing that teens taking antidepressant drugs are more likely to commit suicide (and to be "successful" at completing the act). The dual reuptake inhibitors venlafaxine and duloxetine do seem to have a slight advantage in terms of efficacy over SSRIs and the other antidepressants like the tricyclics, but at least in the case of venlafaxine the suicide risk may negate the added efficacy. For severe depression, especially for those who have no response to an SSRI, some of the tricyclics, like imipramine or clomipramine, may work better and are worth a try. For bipolar (manic-depressive) disorder, lithium is useful for preventing a return of manic episodes.

I went public with accusations about the suicide risk of of antidepressant drugs in 1999. At that time it was widely ridiculed and called a "conspiracy theory." Now, once again, it is emerging as scientific fact. There are so many murder/suicides linked to Prozac and other SSRI drugs that the public would be absolutely shocked to learn the true details of how many people have been killed by these drugs. Remember Phil Hartman, the comedian? He was killed by his wife who was also on these drugs.

Post front-page article by Ron Stein and Marc Kaufman headlined "Depression Drugs Safe, Beneficial, Studies Say: suicide risk Rejected, But Critics Question Validity of Findings, " covers in some detail the results of two taxpayer-funded studies that report to negate the FDA's earlier conclusions about the potentially deadly risks associated with antidepressant use, which moved the FDA to request of the pharmaceutical companies to place "black box" warnings on the psychiatric mind-altering drugs.
As a result, the USPSFT could not determine the balance of benefits and harms of screening for suicide risk in the primary care setting. "8 So, according to the USPSFT, there is insufficient evidence to support the claim that screening reduces the risk of suicide attempts, and it further states that there is insufficient evidence that "treatment" reduces suicide attempts. This is important information when one considers the other goal associated with the nationwide mental health plan.
All kinds of facts and figures are provided by the program to support the need for early diagnosis of mental illnesses that may lead to suicide, and TeenScreen reports: "Research has shown that a significant association exists between these disorders and suicide risk."7 While TeenScreen would have America believe that screening a school-age child will somehow deter the child from future suicidal thoughts and actions, this is at odds with other expert opinions. For instance, the U.S.
Computer-based questionnaires will be used to screen for mental illness and suicide risk. • Columbia University TeenScreen Program provides consultation, screening materials, software, training and technical assistance to qualifying schools and communities. • In return, TeenScreen partners are expected to screen at least 200 youth per year and ensure that a licensed mental health professional is on site to give immediate counseling and referral services for youth at greatest risk.
They always exclude patients who have a significant suicide risk and those who have a diagnosis of manic depressive disorder. So, the patients who are in these trials represent a lower risk of severe depression and suicidality and other abnormal mental responses than do many patients in routine clinical practice. In addition, the trials are very limited in their ability to pick up adverse reactions because they generally only last four to six weeks.

Speaking to an FDA advisory panel convened in 2004 to decide whether to place a prominent suicide risk warning on antidepressants, Healy, who still believes in the therapeutic value of well-monitored SSRIs, delivered a damning set of data. "There have been 677 trials involving SSRIs," he said, "and having helped review all of these, I can let you know that roughly only a quarter of the suicidal acts that have happened in these trials have been reported in the scientific articles that have come out of those trials." There were all kinds of duplicity and institutional leniency at work as well.
But the FDA, already getting an earful about suicide risk and Prozac, was worried about the "public relations" effect of such figures, and wrote SmithKline, telling the company to resubmit its data. As a number of court cases now reveal, SmithKline reran the numbers, giving the drug a more benign profile. Paxil sailed to approval. As Healy noted, "They were only doing what Lilly and Pfizer had already done.

Finally, I want to mention that I warned people about the suicide risk of Prozac back in 1998. Back then, the idea that antidepressant drugs could cause suicides was considered ludicrous. It was fringe information, almost conspiracy theory stuff. Today, we're learning it's 100% true. And if you don't believe me, just ask the parents of all the children killed at the Colombine high school massacre in Colorado: their killers were on antidepressant drugs. Getting the picture yet?

He pointed out that the FDA had failed the public since they had known of the suicide risk for many years. I have entitled this chapter "Tricks of the Trade." I believe you understand why. Within this short story we see two tricks. Relabeling can hide facts that would be quickly seen otherwise. Calling suicidal thoughts and behaviors "emotional lability" allowed the trials to be fded with the British authorities without anyone noticing a problem. The even more common trick is to "count successes only" which will be the first of the tricks of the trade I want you to know about.
But alcoholics are also far more likely to be divorced, have lost their jobs and have major health problems—other suicide risk factors. So let us leave the research on alcoholics alone and simply ask if there is an increase in suicide among those who have simply had some alcohol to drink. The results of this inquiry make it plain that the strong sense of self-preservation we all possess can be dramatically blunted by alcohol. For example, an American study found that a third of all persons committing suicide had been drinking.
The German warnings were ignored by our FDA as were the many research reports suggesting the suicide risk was very real. Describing the situation that existed as of October 1993, Ann Blake Tracy wrote, "Prozac has been out only six years and the most severe emotional side effects did not even come to public or professional attention until February 1990 and yet there are 28,623 complaints registered [with the FDA], a figure higher than any other drug in the history of the FDA."20 Of those registered complaints, 1,885 involved suicide attempts.
Most Americans are very aware of the much reported FDA warnings concerning suicide risk for children on antidepressants. Based on 24 trials the FDA stated that they believed the risk was twice as great for children on antidepressants as compared with those on placebo.18 It was unfortunate that the FDA did not issue a similar warning for adult use as the German drug regulators felt would be necessary if Prozac were ever to be approved in Germany. That was 1985, two years before Prozac began to be prescribed in our nation.
They simply reworked it and published it—with the Remeron and the suicide risk comments removed— under the names of some new "authors."7 This is not a rare event. There are numerous ghostwriting agencies composing and publishing articles in medical journals. As a result of a lawsuit against Pfizer, documents were made public which revealed they had used a New York ghostwriting agency to produce nearly 90 articles for their antidepressant Zoloft.
It noted the BGA's concerns about the suicide risk. It stated, "During the treatment with the preparation, 16 suicide attempts were made, two of these with success. As patients with a risk of suicide were excluded from the studies, it is probable that the high proportion can be attributed to an action of the preparation."15 The "preparation" was, of course, Prozac. So great was the BGA's concern, they told Lilly they could not approve Prozac for use in Germany. That was 1985. Prozac was approved in 1987 in the U.S.

And indeed, the devastating so-called 'withdrawal syndrome' associated with some of the SSRI antidepressants, coupled with an increased suicide risk particularly among young people, again in only some of the drugs are why the pharmaceutical industry has been getting a bad press recently. More money, fewer drugs But it is the dynamics of the markets themselves that expose the public to greater than necessary risk. Intense pressure on companies to increase their share of the big markets has, over the years, developed a momentum of its own, compelling companies to merge if they want to survive.

In his article for the Guardian newspaper entitled "The missing warning on a drug for desperate people, Users of an antidepressant are not yet being alerted to a possible suicide risk," Edward Harriman discusses the company's silence on this serious issue. (http://www.guardian.co.uk/comment/story/0,3604,492305,00.html) He starts with this statement, "It may seem hard to believe that a big drug company would drag its feet over an official request to provide depressed people with warnings that might save their lives. But that is what appears to be happening.

Yet even though more than five hundred suicide attempts had been reported to the FDA as possible drug reactions, and the chairman of the advisory committee believed there should be a label change to at least mention the possibility of a suicide risk, the panel chose to take no action whatsoever. It's not easy to sanely debate drug science amid the crazy politics of mental health.

People respond differently to the various types of antidepressant drugs; and a person's symptoms, age, whether or not they have a physical illness, suicide risk, and response to previous medication may help to play a role in deciding what drugs are given. Types of antidepressant drugs include heterocyclic antidepressants, monoamine oxidase inhibitors and selective serotonin re-uptake inhibitors. An appropriate exercise program is an important part of any treatment of depression. The effects of exercise in improving the symptoms of depression are well-documented.
