| I was astounded by what I heard — and did not hear — about the action of steroids at the cellular level. No one recognized that frequent hypoglycemic episodes caused by tight control offer a possible explanation for damage. When a diabetic experiences a moderate-to-severe hypoglycemic event (low blood sugar), his body releases cortical steroids as part of the survival mechanism. For an indeterminate period of time, these released steroids alter the way in which insulin is metabolized. |

It is responsible for the production of corticosteroids (also called adrenal steroids). Over 30 different steroids have been isolated from the adrenal cortex, including Cortisol and cortisone.
As mentioned in chapter 5, secretion of Cortisol (as well other adrenal hormones) occurs in daily cycles, peaking in the morning and having the lowest values at night. In response to stress, high amounts of Cortisol are released. The body is designed to deal with this, but when stress is frequent or ongoing, imbalances usually result. |

This can happen with some hormonal contraceptives, with some antidepressants and antipsychotics, and with any drug that includes steroids, such as prednisone (usually taken orally) and inhaled steroids used to treat asthma.
2. Reduced absorption of nutrients. Most drugs discussed in this book are taken by mouth. In passing through the gastrointestinal tract, they can bind to specific nutrients before they're able to be absorbed into your bloodstream. |

In an earlier reference, it was also recognized that hypoglycemia also causes changes in the ischemic qualities in the back of the eye.3 Aventis stated that today's insulins can cause a worsening of eye problems; yet none of the insulin cartel has correlated the use of synthetic insulin to eye damage nor to increased depression.4 |

Some powerful drugs that are only used in extreme cases do have side effects, though: certain steroids can affect a child's growth, but are generally not used at anything like sufficient dosage to cause a problem. However, if your child is on steroids and does not appear to be growing at a normal rate, talk this over with your doctor.
The best way of delivering asthma medication is through the lungs, which is why doctors prescribe inhalers of various kinds for children. The design of these is evolving and they are now easier for children to use correctly than they used to be. |

Nobody would fault a pediatrician for giving steroids to an asthmatic child, even though steroids, like most medications, have never received specific pediatric approval from the FDA.) Thought leaders help drug companies get around the no-off-label-marketing rules by doing the promotion for them. A doctor's decision to prescribe off label can be influenced by the recommendation of other doctors or by scientific articles in the medical literature, which the physician may or may not be able to interpret. Companies use thought leaders to reach doctors from both directions. |

The search was on to find reasonable substitutes for the body's own scarce supply of steroids.
Halberg had been singled out to study mice whose adrenal glands had been removed and who were then injected with adrenaline in order to observe the effect on their circulating white blood cells called eosinophils. In ordinary circumstances, adrenaline will set off a predictable seesaw, causing more of the body's natural steroids to be secreted, in turn lowering the eosinophil count. However, in animals or humans without adrenal glands, the count should remain static. |

Once people woke up to the downside of steroids, the drugs lost their luster and fell into disfavor. Don't get us wrong, though. These medications are incredibly valuable, especially for short-term use. People experiencing an arthritis flare-up, a bad sunburn, or a terrible case of poison ivy will benefit immensely from a pulsed dose of corticosteroids. When Joe went deaf in one ear, a course of prednisone restored his hearing. If used cautiously and with respect for their risks, these drugs can be extremely valuable. But using corticosteroids regularly to treat arthritis is a slippery slope. |

41 In a prospective, randomized, double-blind, placebo-controlled trial, 28 patients with idiopathic sudden sensorineural hearing loss were treated with either steroids and oral magnesium (study group) or steroids and a placebo (control group). Compared to the controls, the magnesium-treated group had a significantly higher proportion of patients with improved hearing (>10 dB hearing level) across all frequencies tested, and a significantly greater mean improvement in all frequencies. |

Second, I use steroids, a class of drugs in use for decades with broad anti-inflammatory actions, rather than some of the newer, less broadly acting drugs. If the pain disappears during this therapeutic trial, we would have all the more evidence of an underlying autoimmune disorder, since there is no evidence—either in the literature or in my own experience— that these drugs improve primary FM. To me, improvement on steroids means the FM is secondary to an underlying autoimmune disease. |

However, steroids induce resistance to several anti-cancer drugs (taxol, gemcitabine, cisplatin, 5-FU and gamma-irradiation). So while steroids are beneficial, they also pose drawbacks. [Cancer Biology Therapy 5: Jan 25, 2006]
The 5 alpha reductase inhibitor finasteride (Propecia) has no detectable effect on the production of testosterone by the adrenal gland, which is why it is of limited value in subduing hormone-sensitive prostate tumors. [Journal Andrology 15:298-301, 1994]
Inhibition of the enzyme 5 alpha reductase interferes with the conversion of testosterone to dihydrotestosterone. |

| Ginseng has a strong influence on the immune system and contains substances that are similar to steroids (a type of hormone in the human body) and also Vitamins Bl, B2, and D. Ginseng is therefore recognised as a useful treatment for asthma and respiratory disorders, especially chronic coughs which often accompany asthma, and it is also used for exhaustion, nervous disorders, tuberculosis and circulatory problems.
There are different types of ginseng: Chinese or Asian, Siberian and American. |

Such patients fall through the cracks of medicine because they do not fulfill diagnostic criteria for autoimmune disease, but on occasion such a patient does feel a lot better on steroids. This successful outcome means that the person probably does have an autoimmune disease causing the symptoms.
Back in the 1980s and 1990s, many doctors believed that disabling fatigue was likely due to a problem with the immune system (called immune dysregulation). Because of this, they sometimes prescribed intravenous gamma globulin in patients with severe cases. |
| When patients have autoimmune diseases, on the other hand, doctors often treat patients with very high doses of these steroids taken as pills. The report showing efficacy of hydrocortisone on fatigue was remarkable in that very small amounts of this drug—actually substantially less than the body itself makes—reduced symptoms in a double-blind placebo-controlled trial. Unfortunately, a follow-up trial was not effective. I have tried low-dose hydrocortisone therapy on about twenty patients with no remarkable success. |

NONALLERGIC ASTHMA TREATMENTS
Nonallergic asthma is a chronic problem that needs to be treated somewhat differently than allergic asthma, which may come and go with avoidable triggers and seasonal changes. Chronic asthma sufferers are more at risk for fatalities if they are not treated. |

They also knew that some studies in animals showed that steroids worked to treat sepsis.
So a placebo-controlled, double-blinded study was done in people. On the surface, using a placebo seemed unfair. steroids worked in the test tube and steroids worked in animals. It made sense that steroids would also work in these critically ill people. Why would doctors be willing to withhold a potential life-saving treatment in the interest of a scientific study? But the results of that study were surprising. |

Inhalers are not as invasive as many oral asthma medications, and steroids, over the short term, can be helpful during a severe attack.
Drugs are often pushed while lifestyle interventions like exercise (which are equally effective based on scientific studies) are ignored and in some cases discouraged. As proof, the Web site of the Asthma and Allergy Foundation of America (www.aafa.org) states, "Exercise—frequently in cold air—is a frequent asthma trigger. |

Another: Since it's a steroid, it has the same potential long-term side effects as other steroids, such as cancer and a weakened immune system. Just because something makes you feel good doesn't mean it's good for you (think cocaine, heroin, or pints of ice cream).
Nevertheless, we do believe that DHEA can be effective for boosting energy, stamina, and reduced sex drive. So if you are wiped out or have low libido, and you've been checked out by docs for everything else, maybe— just maybe—low doses of DHEA might help. |

He stated that antioxidants that have good "bio-avaiiability" (are easily used in the lung) may therefore be potential therapies that would not only protect against the direct injurious effects of oxidants, but also may favorably alter the events that have a central role in the development of COPD,4
The progression of COPD is relatively resistant to traditional medical treatment, especially the use of steroids. Obviously, the first task of the physician is usually to help smoking patients to quit. This is not an easy task. |
| Our therapies changed considerably, and we shelved the theophylline-like drugs in favor of anti-inflammatory drugs (inhaled steroids or Intal) as first-line therapy. Research conducted over the past decade has since deduced that the underlying cause of asthma, and most every chronic lung disease, is oxidative stress.'
My children's physical education teacher told me that when she began teaching twenty years ago she would ask the school kids to run a mile. It was no big deal. Now the story is altogether different. When she asks kids to run a mile, she ends up with two pockets full of inhalers. |
| After three weeks of faithfully swallowing many pills and consuming those strange-looking drinks, Liz felt so good that she stopped taking the steroids and nebulizer treatments.
Three months passed, all bringing gradual improvement, and Liz never looked back. She was stronger than she had been in years and exuded a renewed outlook on life. I saw the sparkle in her eyes when she returned from training and caring for her horses. She not only could do the work in the horse barn but also was no longer fearful of suffering from allergic reactions to the hay, mold, and dust. |
| We consulted an infectious-disease specialist, and he placed Liz on intravenous antibiotics, steroids, and nebulizer treatments. Fortunately, within two weeks the pneumonia cleared. Her cough, however, persisted, and she continued on heavy medication for months.
Of greater concern was her fatigue, which was now worse than ever. Liz was out of bed only about two hours a day. Her asthma and allergies were raging and only with luck could she make the walk to the barn to see her horses. Liz was so sick the children took turns staying home from school to care for her. |
