They were always friendly, and over time, we did become close friends with them. In private practice, we joked that a drug rep couldn't see us unless he brought some toys. Drug approval in the United States requires a carefully planned series of steps, sometimes taking as long as twelve years to complete, and it involves billions of dollars and numerous industries and professionals such as chemists, research specialists, medical professionals, lawyers, politicians, marketers, and pharmaceutical reps. It was estimated that in 1998 the pharmaceutical industry spent $24 billion dollars in research and development alone.
Prior to 1997, pharmaceutical reps were never allowed to promote a drug for a treatment other than its prescribed use.12 The new legislation did away with the longstanding prohibition of giving out information on unapproved uses of drugs by the manufacturer. Drug companies now had permission to circulate information on the use of their drug for diseases or clinical situations that the FDA had never approved. Reps could suggest the sale of new drugs for uses with no proof of safety or effectiveness, such as a seizure drug that could also be used as a painkiller.

Also, the importance of consistent re-evaluation has apparently not been stressed to the prescribing physician by the pharmaceutical reps, because close monitoring of Prozac is very rare. In fact, it is amazing to see how often the suggestion is made that a patient may need to stay on Prozac indefinitely - even for life. Doctors receive very little training in medical school on drugs. It would make far more sense for doctors to diagnose and pharmacists to prescribe the drugs since they know so much more about them.
It is definitely the attitude of far too many within the medical community, after being persuaded by pharmaceutical reps that Prozac has few side effects that, "This is a wonderful new drug with no side effects. Here, try it. You will feel great." Then they burst into song, "Happy trails to you...Until we meet again. Happy trails to you...Keep smiling until then." The patient rarely even gets the "call me in the morning" on this one! That may seem slightly theatrical, but it is all too real. The doctor all too often says, "Here is your prescription for the next year.
Do they rely solely upon the pharmaceutical reps to educate them about these new drugs? And how do pharmaceutical companies continue to get away with keeping new research, which may prove damaging to their sales, from the FDA and the public? Why is it so difficult to find a responsible, caring, knowledgeable physician anymore? It actually brings new meaning to the term "butt head" when we learn that proctologists are prescribing Prozac - what an insult to a patient's intelligence. It would also serve as evidence that some doctors don't have a clue as to which end is up.
The big promotional thrust of the pharmaceutical reps is the claim that Prozac has no side effects or fewer side effects than other antidepressants and each succeeding SSRI supposedly has even fewer than the last one. Patients have been so unaware of the effects because of these claims and so confused, shocked and embarrassed through suffering those adverse effects that they have been unable to make their doctors aware.
