Practitioners of manipulation report that it often produces immediate pain relief either in the area manipulated or elsewhere.60 Controlled trials have found that people given spinal manipulation may experience reduction in pain sensitivity of the skin in related areas,61'62 a reduction in joint and muscle tenderness in the area manipulated,63 and a decrease in elbow tenderness when the neck was manipulated.
No research has been published investigating the pain control potential of 5-hydrox-ytryptophan (page 459) (5-HTP), another serotonin precursor that, unlike L-tryptophan, is currently available without a prescription. Vitamin Bu (page 601) has exhibited pain-killing properties in animal studies.29 In humans with vertebral pain syndromes, injections of massive amounts of vitamin J3i2 (5,000 to 10,000 meg per day) have reportedly provided pain relief.30 Further studies are needed to confirm the efficacy of this treatment. Are there any side effects or interactions?
Other preliminary trials have confirmed the effect of DPA in chronic pain control,78 but a double-blind trial found no benefit.79 DPA inhibits the enzyme (page 506) that breaks down some of the body's natural painkillers, substances called enkephalins, which are similar to endorphins. An increase in the amount of enkephalins may explain the reported pain-relieving effect of DPA. If DPA is not available, a related product, D,L-phenylalanine (page 568) (DLPA), may be substituted (1,500 to 2,000 mg per day).

Congress passed a bill (signed by President Clinton) calling for a "Decade of pain control and Research" to begin on January 1, 2001 (Saner et al. 2000). While the undertreatment of pain predominates at present, it is also important to not fall into the trap of thinking that we need more and more different kinds of medication for pain relief.
Instead, it acts as an endorphin shield, battling pain indirectly by helping the body's built-in pain control system grow more powerful. Phenylalanine was first tested against pain in a 1978 study at the Chicago Medical School. Researchers began by timing how long laboratory mice would remain on a hot plate before jumping off. Then they injected hundreds of mice with phenylalanine and again watched to see how long the mice would remain on the heated surface before scurrying off. The amino acid blocked pain in 70% of the mice, allowing them to stay on the hot surface longer.
Fentanyl is commonly used by anesthesiologists and surgeons for pain control. Fentanyl is in the same drug class as morphine and Demerol. Valium is very commonly used in numerous settings to decrease anxiety, for muscle relaxation, and for sedation. Propofol is a newer anesthetic agent, favored by many anesthesiologists for a variety of reasons; it is used for induction and maintenance during anesthesia. Clonidine is not commonly used, but it can be used orally for sleep or sedation in the postoperative period.
The anesthesiologist should be aware of your concerns and should be able to explain to you how pain control will be managed. If you do not wish to remember the procedure at all, also speak to the anesthesiologist, so an IV sedative with good amnesic qualities can be chosen. Note: Surgeries that enter the abdominal cavity usually require paralysis of the abdominal muscles for adequate exposure, and this requires a general anesthetic in which paralytics (muscle paralyzing agents) are used.
While Tegretol (carbamazepine) has been used for pain control in neuropathic conditions and also in other chronic pain conditions for some time, the use of gabapentin is fairly recent. A systematic review of the literature canied out by Mel-legers et al. (2001) found a total of 35 papets involving 727 patients with multiple neuropathic pain conditions that met the criteria for analysis.
