You can almost always get people comfortable, usually without narcotics. But, if you need narcotics, you need narcotics. The vast majority can get pain- free without them. Like you said, when you treat the underlying causes of pain -- and let's talk about those in a moment -- you want to outline the kind of pain. Is it nerve pain, muscle pain, bone pain, inflammatory pain? The book will take people through each kind of pain and how you can tell what's causing the pain. Is it a migraine or is it a regular headache? Then it talks about how to get rid of it naturally. |

Semisynthetic narcotics have little to do with reducing mortality, our primary focus in this book. Yet there is little doubt that these drugs reduce pain, without doubt a laudable goal. "Narcotic pain medications are wonders of modern medicine for patients with serious pain who are under the care of physicians."1 So said the Director of the Substance Abuse and Mental Health Administration (SAMHA) in 2004.
So far, so good.
Unfortunately, there is more. |

This theory seemed to be confirmed when she perked up after an injection of Narcan, a drug that reverses the effects of narcotics. King told McKee her daughter was thirsty and asked a nurse to bring her something to drink. Josie gulped down a liter of juice, the equivalent of more than four cups.
McKee paged Paidas. After consulting with McKee about Josie's response to the Narcan, he wrote in the girl's chart that she was not to be given any more pain medication unless he was consulted. Then he left for surgery. About an hour later, Josie got another shot of Narcan. |

The Egyptians used it to simulate opium-like hallucinations without narcotics. Yohimbe is a popular herbal product marketed as a sexual stimulant by enhancing blood flow. The plant maca, grown in Peru, is marketed to increase energy, stamina and sexual ability. The Indian herb tribulus is said to increase the body's natural testosterone levels and improve sex drive. Eurycoma longifolia, grown in Southeast Asia, is touted as a powerful aphrodisiac.
Manufacturers can throw together any combination of these allegedly ancient ingredients to make up their own secret formula. |

A court date was set for four months down the road, and his lawyer and I worked out a treatment plan — each week he had to take two drug tests and attend a meeting at Alcoholics Anonymous and one at narcotics Anonymous. He knew he had to stay clean at least until his court appearance, but he started to crave cocaine. His lawyer told him he'd probably get the maximum sentence of three years in jail, and he desperately wanted help. Dealing with his cravings was our first order of business, and I told Rusty that exercise could have a huge impact. |
| I wrote a letter to the judge, and Rusty was put on probation rather than given jail time, on the condition that he would continue drug testing, narcotics Anonymous, and counseling at college. He took his DDR setup with him and continued doing it every day for a while. Then he joined an intramural soccer team and started going to the gym.
Exercise was a conduit for shifting Rusty's focus to a more productive life. I see exercise as a way of offsetting the feeling of hopelessness and uselessness that a lot of drug users have, and that certainly was a factor with Rusty. |

Conventional treatment advocates pain relief medications, such as nonsteroidal antiinflammatory drugs (such as ibuprofen or aspirin) or narcotics like Percocet. Diuretics are also sometimes prescribed for prevention of calcium oxalate stone formation. Besides medication, lithotripsy is an ultrasound treatment that breaks the stones into pieces that are small enough to pass in the urine. This procedure has largely replaced surgery as the preferred method for stone removal. (Note: If you're going to get surgery, it might be worth it to try acupuncture. |

As a nation, we take far too many narcotics, and it should come as no surprise that many of us, more every year, are addicted.
We will discuss antidepressants, the next most often prescribed category of drugs, in the following chapter. Suffice to say, they don't work very well, let alone save lives.
Statins, in third place, are ineffective in the prevention of cardiovascular problems, but they are effective in treating recurrent disease— an accomplishment we dare not overlook. Next in prescription activity are ACE inhibitors, which, surprisingly (scandalously!) work no better than diuretics. |

After his surgery, Casarella wrote about his experience in a letter to the editor of the journal Radiology, describing it in the terse, unemotional language of medicine, which nonetheless conveyed his sense of helplessness and the pain: "I woke up five hours later with a Foley catheter, a chest tube, a central venous catheter, a nasal oxygen catheter, an epidural catheter, an arterial catheter, subcutaneously administered heparin, a constant infusion of prophylactic antibiotics, and patient-controlled analgesia with intravenously-administered narcotics. |

We've already considered semisynthetic narcotics; our discussion of antidepressants awaits the following chapter. Therefore, we turn our attention to...
Statins
Statins are a category of drug9 that slow the body's ability to produce cholesterol and increase the liver's ability to remove low density lipids (LDLs), so-called bad cholesterol. We pose three questions: (1) do statins lower blood cholesterol? (2) do statins prevent heart attacks? (3) are statins effective in the treatment of heart disease?
First, there is widespread agreement that statins do lower blood cholesterol. |

There are certain rote recommendations that I was aware of: tell them to go to Alcoholics Anonymous and narcotics Anonymous, to go to "90 in 90" (ninety meetings in ninety days), to stay away from the "people, places, and things" that might trigger the desire to use drugs or alcohol—but other than these simple directives, I felt completely stumped. I had no tools with which to work.
What I learned at the training gave me a framework to better understand my patients and their reluctance to change. |

No doubt: synthetic narcotics are a threat—a growing threat at that—to the public health. Unfortunately SAMHA's analysis lead us away from facing the problem, let alone finding a solution.
TRENDS
We need to throw out some numbers.
As Jonas Salk was putting the final touches on his vaccine in 1954, most drugs were sold over-the-counter (OTC). The best selling were Bufferin (Johnson & Johnson) and Geritol (Merck). By 2005, more than 100,000 preparations with 1,000 active ingredients were sold OTC. Annual sales exceeded $17 billion.4
Maybe there is something to be said for the 1950s nostalgia! |
| When diverted from their legitimate use," continued the Director, "[these drugs] are highly addictive narcotics that the body perceives exactly as if the person were taking heroin."
I showed the quote to Fran first thing next morning. "They are hiding more than they are revealing."
"I can't talk before my first cup of coffee," she replied in a weak voice.
I was silent.
"What do they mean by 'legitimate,' Fran asked, ten minutes later, her large mug half empty.
"I don't know, but our government actually maintains that marijuana is the most commonly used illegitimate drug. |

I believe the odds of long-term recovery can be greatly improved by including dietary modification and supplements. (More further on.) In fact, Bill Wilson, the founder of AA, advocated the use of high-potency B vitamins in treating alcoholism.
Can an alcoholic ever go back to drinking in moderation? It's a controversial idea. AA contends that a person who is an alcoholic will always be an alcoholic and should never again drink alcohol. |

What if all narcotics and opiates were legalized (decriminalized) at this very instant? no legislative process, no getting ready for years, no
147 advance warning for producers and users, let alone law enforcement. What would happen? Just as importantly: what would not happen? Can we learn anything from history, specifically from the experience of alcohol prohibition in the United States? Can we learn anything from comparative studies, specifically the experiences of other countries that have already selectively decriminalized these drugs? |

On hearing the story of this woman's medical woes, Groddeck concluded that she was a person with a deep "resistance to becoming well." He launched three months of intensive psychoanalytic-style investigation, and this gradually produced a second story, the story behind the medical symptoms. It emerged that the woman was a Christian who—in a time when anti-Semitic sentiments in Weimar Germany were on the rise—had married a Jewish man. |

Many doctors go from prescribing over-the-counter pain medicines like ibuprofen to prescribing narcotics like Percocet when pain continues. This is not good medicine. Percocet has no role in the treatment of chronic pain. If you are on your third visit to a doctor and either your pain remains or you are taking Percocet or another such narcotic, you need a referral to a pain management center. They will certainly know how to help you.

Many medications can also cause constipation, including antihistamines, blood pressure medications, water pills, calcium, iron supplements, and narcotics. Review the medications you are taking.
Diet and exercise: General lifestyle measures to relieve constipation include exercising daily (for example, about 20 minutes of rapid walking a day), drinking at least 1.5 liters of water a day, and including plenty of fiber in your diet (ideally about 30 g a day). Take 2 tbsp of psyllium husks a day and eat at least 5 portions of fresh fruit and vegetables a day. |

More than one-third (38%) of those who unintentionally overdosed took at least two acetaminophen preparations simultaneously. Almost two-thirds (63%) used narcotic-containing compounds in addition to acetaminophen, according to the study.
One-third of the patients (35%) who had acute liver failure died, and 27% died without ever undergoing a transplant.
A SIGNIFICANT PROBLEM
"This is not a problem of epidemic proportions, [but] there is still a significant problem," says Larson.

| Medications, such as antibiotics, narcotics, barbiturates and antihistamines, can trigger a fever as well. Cancer, especially leukemia or lymphoma, can cause a persistent fever of 100°F to 101°F.
Myth 3: Fever should always be treated.
Most of the time, you don't need to do anything to reduce a fever. Unless it goes quite high (above 104°F in an adult), it causes no permanent damage—but you will feel uncomfortable. Some doctors suggest letting a fever run its course. That way, you won't undermine your body's natural defense process. |

Comfort Deficiency Syndrome could be treated with powerful narcotics that take your mind off the bumpy ride. (Never mind the fact that it would also make you a terrible driver. One-third of auto accidents today are already caused by people on medication...)
In all this, the true underlying cause of these problems -- the flat tire -- would be utterly ignored. Why fix the flat tire when they can make so much money managing the "symptoms" of those diseases over many years? |
