I also gave some thought to the fact that nearly every study on immune function had looked within blood. That left out a whole range of other possibilities. One of the first studies I ever did showed that patients with fatiguing illness—produced by CFS or MS—also had cognitive problems. As a neurologist, I immediately came up with the idea that some patients with fatigue might actually experience their symptoms due to a brain problem. It occurred to me that the place to look for immune abnormalities might be the fluid that bathes the brain—spinal fluid—rather than blood.
What happens at night might also be well worth studying and nocturnal alterations in immune function might produce chronic fatigue and pain by disturbing sleep. The immune system is known to regulate sleep via a balance between sleep-producing and sleep-disturbing cytokines. Perhaps some patients had a disease of cytokine regulation in which the normal balance of the different kinds of cytokines was tilted in favor of sleep-disturbing cytokines, resulting in night after night of poor sleep.
However, research done to try to replicate these early reports, including our own, found relatively little difference in the immune function of patients and of healthy people. One explanation for this result lay in the fact that data from extremely sedentary patients were compared to data from active healthy people; these differences in activity or fitness alone can explain much of the difference between patients and controls.
