Melatonin (page 555)
Years ago, a preliminary study suggested that melatonin may help stabilize the condition of some people with advanced cancers.67 Since then, Italian researchers have been investigating the effects of melatonin in cancer patients, often with partial success 68,69'70'71,72'73'74,75'76'77'78'79
Patients with advanced prostate cancer who had previously not responded to drug therapy (ttiptorelin) were given melatonin plus triptorelin in a preliminary trial.80 PSA scores, a marker of disease progression, fell (i.e., improved) more than 50% in 8 of 14 patients. |

The results of several clinical trials suggest that gamma oryzanol (page 525) supplementation can help people with gastritis and other gastrointestinal complaints. In one study, people with chronic gastritis were given 300 mg of gamma oryzanol per day.19 After two weeks, 23% of people taking gamma oryzanol reported that it was "extremely effective" and 55% rated it as "moderately effective." Another study produced similar results: People with various types of gastritis received 300 mg of gamma oryzanol per day. |
| Are there any side effects or interactions?
Borage seeds contain small amounts of liver toxins called pyrrolizidine alkaloids (PA). However, testing has not demonstrated the presence of the alkaloid in the seed oil.20 Most commercially available borage seed oil is, therefore, likely to be PA-free and presents no risk of PA toxicity. Minor side effects from borage oil use can include bloating, nausea, indigestion (page 260), and headache.21

Type-2 diabetes is a reversible health condition. Since the condition is defined by measurable symptoms, once a person restores normal sugar metabolism and the symptoms disappear, that person is by definition no longer diabetic. Diabetes is not genetic in origin, nor is it an inevitable condition for individuals whose parents expressed symptoms of diabetes. This potent antioxidant may be of particular benefit in polycystic ovary syndrome (PCOS), a condition that involves insulin resistance, elevated testosterone levels, and infertility. In one study, researchers asked twenty women with PCOS to take 600 mg of NAC three times daily for four weeks. By the end of the study, the women's insulin levels had decreased by one-third, and their testosterone levels dropped by more than half. Next we consider the problem of gallstones, a sometimes painful condition, but one that is not often associated with serious morbidity or mortality. To justify preventive surgery, the benefit must outweigh the risk. Traditional surgery was invasive and associated with hospital stays and long recoveries. In 1989, a far less invasive procedure, called laparoscopic cholecystectomy, was developed. Five years later, it accounted for 90% of all gallstone operations. Case fatality rates declined. There were risks, particularly bile duct injuries, malpractice claims for which increased fivefold. |
| In a culture such as ours, one that medicalizes the human condition, it will always be difficult to think about, let alone evaluate, the necessity or appropriateness of surgery.
