The definitive test is a biopsy from the surface of the small intestine.
A flexible telescope (endoscope) is usually passed through the mouth into the stomach and upper intestine so that the lining can be inspected and a biopsy taken. This process takes only a few minutes and is often made easier and less uncomfortable by giving the patient a mild sedative beforehand. Alternatively, a biopsy can be obtained by swallowing a special capsule on the end of a narrow tube. |

When there is doubt about an earlier diagnosis, or the changes seen on biopsy are uncertain, another biopsy may be advised after a person has deliberately eaten gluten for a period (this is called a 'gluten challenge').

The diagnosis of Sjogren's Disease (instead of merely Sjogren's Syndrome) is made when doctors either find antibodies circulating in the blood directed to destroying these glands or when microscopic examination of these glands following a biopsy turns up evidence of their destruction by immune cells. A biopsy of a minor salivary gland is accomplished by making a small incision in the inside of the lower lip and removing one of the many bead-sized salivary glands found just under the surface of the lip in that location. |

That's because my tests showed that at least some patients who complain of chronic fatigue have clearly identifiable physical differences on lip biopsy from those of healthy patients. Specifically, a careful microscopic evaluation revealed that a third of my patients with chronic fatigue showed immune cell attack of some of their salivary glands. Why is this study so important? Two conclusions provide the answer: Quite a few patients with severe, medically unexplained fatigue and sicca have undiagnosed Sjogren's Disease, an autoimmune disease, as the cause of their problem. |

Assume almost the worst, that this biopsy is positive. If the tumor appears aggressive, radical treatment is indicated; if not, the question is this: What should be done? Should the patient still be subject to prostatectomy or radiation? The first leads to difficult surgery and recovery; both cause urinary and sexual dysfunction in a substantial proportion of men. Cancer surgery may have one or more of the folloxving goals:
• to determine whether a cancer is malignant by biopsy examination.
• to remove a cancerous growth from the body.
• to learn whether malignant cells have spread to other parts of the body.

And if the biopsy is positive, I ask them to see a rheumatologist. The rheumatologist and I can then work together to help the patient feel better. This may seem like a lot to go through to get to the bottom of chronic fatigue, but it's obviously worth it. From my point of view, it is the doctor's job to walk down this path with you in order to identify any potentially treatable medical causes of your symptoms.

Women on alendronate were found to take up to two years to heal after a fracture and had markedly suppressed bone formation on biopsy. It looks like bisphosphonates have a modest effect in reducing fractures during the first five years of treatment. In the early and mid-1980s, the few women who experienced this problem saw a gynecologist, and most had an endometrial biopsy. | Carcinogenic Effects: Juniper tar has been investigated in human skin explants from biopsy samples and skin and lungs of mice. Direct evidence of formation of potentially carcinogenic DNA damage was seen in both human and mouse tissue (Schoket et al, 1990; Phillips, 1989).
Ocular Effects: Juniper tar may be irritating to the eyes and cause conjunctivitis (Gosselin et al, 1984).
Renal Toxicity: Prolonged use or overdose may result in renal damage. Alpha and beta-pinene (monoterpenes) content in Juniper berry oils is associated with renal toxicity. |
