Our Daily Meds: How the Pharmaceutical Companies Transformed Themselves into Slick Marketing Machines and Hooked the Nation on Prescription Drugs

GlaxoSmithKline included these instructions on a website where it promoted wellbutrin with a brand logo resembling a shining yellow sun. The website also featured the photo of a smiling woman, who appeared about Karen's age, standing next to a tranquil lake. The tagline: "I'm ready to experience life." Wellbutrin had killed Karen Garcia and almost got away scot-free. It took three independent forensic experts and a yearlong investigation to determine what had really killed Karen. Most families don't get the benefit of such careful determination of what caused their loved one's death.
The seizure had been brought on, he said, by the antidepressant she was taking, a drug called wellbutrin. When her body began to convulse, Karen appeared to have fallen from her chair, pulling the computer and fan from her desk. She struck her head on the desk and the frame of her bed. When she landed unconscious on the floor, the coroner said, her neck was at such an awkward angle that she could not breathe. The investigators determined Karen had been taking 150 milligrams of wellbutrin for three weeks but had increased her dose to 300 milligrams a few days before she died.

Your Symptoms Are Real: What to Do When Your Doctor Says Nothing Is Wrong

If the depression is unchanged, I will taper off this drug and then, midway through doing this, I will start a drug from a different class, such as Cymbalta or wellbutrin. If the Lexapro has improved mood in part, I will consider adding a second antidepressant. However, treating depressive symptoms with more than one medicine is often something a primary-care physician is uncomfortable in doing. In my own practice, I often send such patients to a psychiatrist expert in the use of brain-active drugs for help in treating depression.

For example, switching from an SSRI to wellbutrin (bupropion) seems to help alleviate arousal and interest issues that are common in people who take SSRIs. So does switching from a beta-blocker for high blood pressure to an angiotensin receptor blocker such as losartan or valsartan. YOU Tip: Drink This. Research shows that Rhodiola rosea, when consumed as a tea or with a light alcohol like vodka, can aid with erectile dysfunction and improve prostate function. To make tea: Cut fine 5 grams of Rhodiola rosea roots.

So Ingram commissioned a series of studies to show that wellbutrin was not only safe, but had fewer side effects than the SSRIs being hawked by Lilly, Pfizer, and SmithKline, with far less sexual dysfunction. He then discharged a corps of medical advisers — essentially salesmen with medical training or even M.D. degrees — to chat up leading mental health specialists, whose prescribing patterns are closely tracked by fellow experts. The tactic worked, and wellbutrin soon took its place in the apothecary of modern psychopharma-cology.

Arif Khan, a psychiatrist and researcher in Washington state, used the Freedom of Information Act to obtain the FDA clinical trial database for antidepressant drugs, which included Prozac, Zoloft, Paxil, Serzone, Remeron, wellbutrin, and Effexor. Khan examined 45 trials, involving about 9,000 patients, and found there was an average symptom reduction of 44 and 48 percent in patients treated with the drugs, and 36 percent with patients treated with placebo after eight weeks of treatment. At the end of the day, an unimpressive difference, unworthy of the hype.
Depakote smelled nice, like vanilla soda, and wellbutrin like rotten eggs. Giving out the pills to my clients, I wondered about their origins: who invented them and how, and who named them; where are they manufactured? (As overwhelmingly ingested by Americans as they are, a good portion of drugs are actually manufactured off the U.S. mainland, a fact that Big Pharma keeps quiet. There are now seventy-five plants in India approved to manufacture drugs for the American market, the most of any nation save for the United States itself.81 Ireland is a key manufacturing site, as is Puerto Rico.
Paxil: "Your Life Is Waiting." Wellbutrin: "I'm Ready to Experience Life." Effexor: "The Change You Deserve" Prozac made it acceptable for drugs to be, simply and solely, lifestyle agents, paving the way for Viagra (slogan: "Get Back to Mischief") and Cialis ("If a Relaxing Moment Turns Into the Right Moment, Will You Be Ready?").
They were, in order, Lexapro, Cymbalta, Zoloft, wellbutrin [an antianxiety agent], Effexor, and the illness of depression itself.)30 Among people who have visited a medication site, depression is by far the most researched medical condition, with 2.9 million unique visitors over a three-month period in 2006. The next most researched conditions? Bipolar disorder and insomnia.31 To further the ascent of Prozac et al. it has helped immensely that there has been a simultaneous barrage of media messages and images in the last decade informing the public that behavior is biologically dictated.
High-profile confessors of recent years include Jane Pauley, whose revelation of bipolar disorder in a book and TV special was timed with the launch of her daytime talk show, and three-time Grammy winner Shawn Colvin, whose tour was sponsored by GlaxoSmithKline, the makers of the folksinger's antidepressant of choice, wellbutrin XL. The favored confessional platform is The Oprah Winfrey Show, where celebrities like Colvin and Linda Hamilton, the tough female action star of The Terminator movies, divulge their inner torments on shows called "Depressed, Mentally 111, and Famous.

If no success is achieved after a few drugs in the same class are tried, then it may be time to move on to another category. Bupropion (Wellbutrin) may offer certain advantages over other SSRI-type drugs. For one thing, it is far less likely to interfere with sexuality. Some have even reported that it restores libido.

Primal Healing: Access the Incredible Power of Feelings to Improve Your Health

Doctors will offer Zoloft, Prozac, wellbutrin, Paxil—the list keeps growing—to help the patient feel better, and sometimes they do. They may stop drinking or bingeing or then may finally be able to relax. Is that better? Of course, but the patient pays a big price: the drugs perpetuate a deep repression by widening the gap between the imprinted experience and the thinking mind, keeping the patient unconscious of his feelings. This may possibly give rise to serious disease later in life. The major point of all of the previously mentioned drugs is to help repression, to block feelings.

An added attraction at the trailer was a visit from Bobby Labonte, the driver of the No. 18 wellbutrin car, and his crew chief, Michael "Fatback" McSwain. NASCAR lovers are among the most dedicated of sports fans. They are known to show their loyalty by buying the products sponsoring their favorite drivers. Mark Martin and his No. 6 Viagra car had fanatical fans all over the country, including Iowa. "I cried last week when he won the race," wrote Kathy Richey of Osage, Iowa, on a guestbook that Mark Martin maintained on his website in 2005, along with photos of his Viagra mobile.
Experience Life," proclaimed the promotional brochure for the antidepressant wellbutrin that I picked up at an Iowa pharmacy. The brochure showed a scene that could have been the cover of a romance novel. A handsome man swept a young, slender woman off her high heels, just as she stepped off a train. A television ad on the 5:30 p.m. news used tactics more like those employed by Victoria's Secret, the retailer of women's lingerie. This ad, for a drug called Levitra, promised improved "erectile quality." The ad had nothing to do with treating an illness.
The investigators determined Karen had been taking 150 milligrams of wellbutrin for three weeks but had increased her dose to 300 milligrams a few days before she died. These doses were not out of the ordinary. Karen was taking the drug just as directed by GlaxoSmithKline, the drug's manufacturer. The company's thirty-six-page prescribing instructions advised that the usual adult dose is 300 milligrams, which can be increased to as much as 450 milligrams. On page nine of those instructions, GlaxoSmithKline also warned the drug can cause seizures, even at the doses Karen was taking.
At some point the doctor switched the Paxil prescription to one for wellbutrin, another antidepressant. But the drugs made Aaron feel worse, just as they had Peter. His appetite disappeared, and he got so skinny that Sandy could see the sharp outlines of his bones in his back. 'Tm sure it was the meds," Sandy said. For years life in the farmhouse surrounded by pines and maples revolved around the pills. After her husband died, Sandy began taking Prozac. She had started tensing her muscles, she said, and having panic attacks.

Atypical antidepressants like wellbutrin can overcome those sexual side effects. The MAOIs work well, especially for atypical depression (increased sleep and appetite), but most people don't like the fact that while taking them it is necessary to avoid wine and cheese. In general all antidepressants are equally effective, so the choice of antidepressants is based on side effects and costs.
The antidepressants that don't have this side effect, like wellbutrin, can be given in addition to an SSRI or substituted for it. Other antidepressants without sexual side effects include Remeron and Desyrel, drugs not in the SSRI class. SSRI treatment is also associated with about a threefold increase in the risk of gastrointestinal bleeding. Although for the average patient there typically is not an increased risk of bleeding, if you are taking aspirin or another NSAID you should be on another medication like Prilosec to protect your stomach.
There are rare cases of seizures with high doses of wellbutrin. Drugs with mixed actions are trazodone (Desyrel) and maprotiline (Ludiomil), which have less-severe anticholinergic side effects and effects on the heart and blood pressure than Desyrel. In rare cases Desyrel can cause priapism (extended painful erection that requires emergency treatment). Nefazodone (Serzone) has both serotonin reuptake inhibition and postsynaptic serotonin receptor blockade properties.

The tactic worked, and wellbutrin soon took its place in the apothecary of modern psychopharma-cology. Ingram also caught a break when physicians at a local veterans hospital found that depressed vets put on wellbutrin were more likely to stop smoking. For them it created a whole new state of health! That allowed Glaxo to win another indication for the drug, beefing up Ingram's arsenal of health creation. In all of this activity, Ingram was learning one thing: it didn't matter how good your drug was if doctors didn't know about it, or if they had gotten a bad first impression.

The (5) anticonvulsants (Depakote, Depakene, Topamax Tegretol, Neurontin, etc.) as a group are not as dangerous or deadly as any of the drugs of groups 1, 2, 3, 4 and 6, but all are foreign compounds, poisons, and should never be used as mental health treatment in medically normal children.

The stages are as follows: STAGE 1: Monotherapy (single drug): the physician may choose one mind-altering drug from the approved SSRIs (Prozac, Paxil, Zoloft or Celexa), or prescribe another antidepressant not specifically a SSRI, such as wellbutrin, Serzone, Effexor or Remeron. STAGE 2: Monotherapy (single drug): the physician may choose any one of the mind-altering drugs listed in Stage 1 or may choose a tricyclic antidepressant (TCA) such as Tofranil, Anafranil, Norpramin, Pamelor or Elavil (all of which are also sedatives).

Norman Hoffman, the head of Student Mental Health Services at McGill University in Montreal, started asking that question when he noticed more and more students, especially those from the United States, taking what was known as "the California cocktail: Neurontin (the anti-epileptic prescribed off label for, among other things, bipolar disorder), wellbutrin (an antidepressant from Glaxo), and Ritalin." Typically, he says, students will report that, since taking the drugs, they are "better." But when he looked at the student more closely, he says, "They are not 'better.

If you start to feel extremely restiess, agitated, anxious, or can't sleep when you start taking wellbutrin, you may need to discontinue it. You may be sensitive to it and prone to delusions, hallucinations, psychosis, confusion, or paranoia with continued use. Take wellbutrin in equally divided doses, three to four times a day, in order to minimize risk of seizure. What Are the Interactions with Other Drugs? Bupropion may increase levels or prolong the effects of the drug levodopa, making adverse effects more likely.

You are switching from monoamine oxidase (MAO) inhibitors to wellbutrin. At least 14 days should elapse between your last dose of MAO inhibitors to the first of wellbutrin. Don't take this drug with MAO inhibitors. • You have recently had a heart attack or any other kind of unstable heart disease. Bupropion should be used with care. • You are taking other antidepressants or antipsychotic drugs. If you have had head trauma, or have any history of seizures, you are much more likely to experience seizures as a side effect of this drug.

We came out with a prescription for wellbutrin [bupropion, an antidepressant not related to tricyclic, heterocyclic or SSRI antidepressants; safety and effectiveness in patient below 18 years of age not established]. At the time I didn't really feel any reason to doubt or second-guess the physician. I always felt physicians were next to God, not realizing they are human and can make mistakes just like anyone else. At first Shaina responded well to the medication and her attention and focusing in school showed improvement.
Then Ritalin was discontinued and wellbutrin (bupropion, another antidepressant never approved by the FDA for use in children) added. Adderall was removed and Zyprexa (a powerful anti-psychotic never tested or approved for use in children either) added. The more medicine they used, the worse he got. He'd be fine for a few weeks, but then it was like uncaging a monster. He started screaming and woke up in the middle of the night hearing voices.
Effexor, Wellbutrin; (5) anticonvulsants (Depakote, Depakene, Tegretol, Neurontin, Topamax, etc.); and (6) antipsychotics (Thorazine, Mellaril, Risperdal, Seroquel, Abilify, Clozapine, Clozaril, Ge-odon, Haldol, Loxitane, Navane, Orap, Zyprexa, etc.). Not only is informed consent a right, it should be every person's duty to themselves, their children and parents (when they act for their children, parents or others) to insist on having all the facts they need to make a fully informed decision regarding their treatment.
