| | First attacks usually occur at night and are usually triggered by a specific event, such as dietary excess, alcohol ingestion, trauma, certain drugs (mainly chemotherapy drugs, certain diuretics, and high dosages of niacin), and surgery.
Causes
Gout is the result of increased synthesis of uric acid; reduced ability to excrete uric acid; or overproduction and underexcretion of uric Acid. | | Celery and cherries appear to be very effective in lowering uric Acid levels and preventing attacks of gout. Celery contains the compound 3-n-butylpthalide, while cherries are a rich source of flavonoids. Both of these compounds are beneficial in gout via several mechanisms, including the ability to inhibit the formation of uric Acid by inhibiting the enzyme xanthine oxidase.
Other Recommendations
The pain of an acute attack of gout is absolutely excruciating. In this situation, heroic measures are clearly appropriate. | | SAFETY
Turkey is not commonly an allergenic food; however, gallbladder attacks have been attributed to turkey allergy. Turkey contains purines. Since purines can be broken down to form uric Acid, individuals with gout should avoid or limit intake of turkey. See page 723 for more information.
Since turkey contains small amounts of oxalates, individuals prone to calcium oxalate-containing kidney stones should limit their consumption of turkey. See page 787 for more information. | | Clinical studies have shown that consuming the equivalent of Vi pound of fresh cherries per day is very effective in lowering uric Acid levels and preventing attacks of gout.
HOW TO SELECT AND STORE
The cherry season is brief, slightly more than three months long. Bing cherries usually appear at the end of May, peak in June and July, and are available through August, while Lamberts and other sweet, dark cherries arrive in mid-August. Varieties that appear earliest and latest are not as sweet and soft as Bings. | | Pork is not considered a commonly allergenic food, although gallbladder attacks have been associated with pork allergy.

| I Panic attacks may be caused by a deficiency of alpha-linolenic Acid, an essential omega-3 fatty Acid found in high concentrations in flaxseed oil. In one study, three out of four people with a history of agoraphobia (fear of going outdoors) improved after two to three months of taking 2 to 3 teaspoons of flaxseed oil daily. Physical signs of a deficiency of alpha-linolenic Acid include dry skin, dandruff, brittle fingernails, and burning sensations in the skin.
J Some authorities in natural medicine suggest taking supplemental gamma-aminobutyric Acid (GABA) to prevent anxiety attacks. | | Obesity increases the risk of repeat gout attacks. (See overweight in Part Two.)
CONSIDERATIONS
Ll Allopurinol (Zyloprim) reduces uric Acid formation. It is a highly effective drug but it can produce severe side effects, such as fever, generalized rashes, breakdown of red blood cells, and liver and kidney problems. Another drug, colchicine, derived from the herb autumn saffron, eases acute gout attacks and prevents future attacks. While often quite effective, it too can have serious side effects.
Ll Steroids may be prescribed during acute attacks. | | LJ Many studies have found a link between asthma attacks, which may be immediate or delayed, and food allergies. In children, foods that are most likely to trigger immediate attacks are eggs, fish, shellfish, nuts, and peanuts. Foods most commonly associated with delayed attacks include milk, chocolate, wheat, citrus, and food colorings. (For more information, see Food Allergies under ALLERGIES in Part Two.)
—J Hypochlorhydria, or deficient production of gastric Acid in the stomach, has been linked to asthma in both children and adults, as well as to a higher incidence of allergies. | | Eating half a pound (225 grams) of canned or fresh cherries a day has been shown to be a very effective means of preventing gout attacks. When fresh cherries are out of season, use cherry extracts or concentrates in tablet or tincture form, available from many health-food stores. It is likely that cherries inhibit compounds that cause inflammation. This reduces inflammation after uric Acid builds up in a joint.
Ll To flush uric Acid from the body, drink no less than eight glasses of water daily.
Avoid alcohol, since alcohol increases uric Acid production. | | Taken as a preventive, it can also reduce the frequency of vomiting with migraine attacks.
LJ Avoid foods containing the amino Acid tyramine. These include anchovies, beer, hard cheeses, chocolate, corned beef, dried meats, fava beans, fermented beans such as miso and soy sauce, lima beans, pickled herring, red wine (white wine can be used in moderation), sardines, sauerkraut, and yeast. Although a single serving of a tyramine-containing food such as chocolate may not provoke a migraine, excessive consumption of these foods can increase the frequency of attacks. | | In children, foods that are most likely to trigger immediate attacks are eggs, fish, shellfish, nuts, and peanuts. | We had hoped that taking vitamins B6, B12, and folic Acid would lower the risk of heart attacks and strokes. To our disappointment, those hopes were dashed.
Studies published in the New England Journal of Medicine conclusively demonstrated that taking B vitamins did not protect high-risk patients from heart attacks or other "major vascular events." They may even pose a hazard in the relatively high doses used in these prevention trials. | | High levels of urate (uric acid) in the blood can cause gout attacks. The crystals precipitate around joints (especially in the big toe) and cause severe inflammation and excruciating pain.
The investigators reported in the Journal of Nutrition: "The decrease in plasma urate after cherry consumption supports the anti-gout reputation of cherries. The trend toward decreased plasma concentrations of the inflammatory markers CRP [C-reactive protein] and NO [nitric oxide] adds to the in vitro evidence that compounds in cherries may inhibit inflammatory pathways. | | Migraine attacks, especially migraines without aura, are about twice as common during the first few days of menstruation as at other times of the cycle.372
Knowing when the migraine is likely to strike can allow for a preventive strategy. Women susceptible to menstrual migraines are often advised to take a nonsteroidal antiinflammatory drug (NSAID) such as mefe-namic Acid (Ponstel) for several days prior to the expected start of menses and continuing for a few days.373 An OTC NSAID such as naproxen (Aleve) or ibuprofen (Advil, Motrin) might also work. | | The researchers had expected that women who adopted the more virtuous low-fat, high-veggie diet would be protected from heart attacks, strokes, breast cancer, and colorectal cancer. But despite the large number of women participating, the differences in the results of the two groups were not statistically significant.434
For cardiovascular disease, the investigators found a trend favoring survival in women who ate the least amount of saturated fat and trans fatty acids (and the most vegetables). |

FIGURE 1 Glucose Breakdown to Pyruvic Acid panic attacks is an elevated blood lactic Acid level and an increased lactic Acid to pyruvic Acid ratio. Lactate (the soluble form of lactic acid) is the final product in the breakdown of blood sugar (glucose) when there is a lack of oxygen.
To illustrate how lactic Acid is produced, let's take the classic example of the exercising muscle. Muscles prefer to use fat as their energy source, but when you exercise vigorously there isn't enough oxygen, so the muscle must burn glucose. | | Scores continued to
Reducing Lactate Levels
As pointed out previously, increased lactic Acid levels may be an underlying factor in panic attacks and anxiety. Our goal is to prevent the conversion of pyruvic Acid to lactic Acid and to improve the conversion of lactic Acid back to pyruvic Acid. Nutrition appears to play a key role in achieving this goal. According to Melvyn Werbach, M.D., author of Nutritional Influences of Mental Illness} there are at least six nutritional factors that may be responsible for elevated lactate levels or lactic Acid to pyruvic Acid ratios:
1. Alcohol
2. | | Consuming one-half pound of fresh or canned cherries per day has been found effective in lowering uric Acid levels and preventing attacks of gout.
Chronic hemolytic anemia
Cytotoxic drugs
Psoriasis Increased synthesis Increased breakdown of purines
Fructose ingestion or infusion

Decreased kidney clearance of uric Acid (primary)
Intrinsic kidney disease Decreased kidney clearance of uric Acid (secondary) Functional impairment of kidney function Drug-induced (e.g., thiazides, salicylates, etc.) Increased lactic Acid (e.g. | | In the same study, those who received 23 grams of linoleic Acid demonstrated reduced frequency and severity of acute attacks, even though the study was only twenty-four months in length.
There appears to be a strong rationale for supplementation with flaxseed oil as well as consuming more fish rich in eicosapentaenoic Acid (EPA) and docosahexanoic Acid (DHA). This recommendation is consistent with Swanks protocol, which included the liberal consumption of fish and supplementation with cod liver oil, a rich source of EPA and DHA. | | By following the dietary guidelines given in the chapter A Health-Promoting Diet, as well as the recommendations for nutritional supplementation given in the chapter Supplementary Measures, you will provide your body with the land of nutritional support it needs to counteract the biochemical derangements found in patients with anxiety and panic attacks.
Flaxseed Oil and Agoraphobia
It has been suggested that patients who get panic attacks may suffer from a deficiency of alpha-linolenic Acid, the essential omega-3 fatty Acid found in high concentrations in flaxseed oil. | | Subsequent attacks are common, with the majority of gout patients having another attack within one year. However, nearly seven percent never have a second attack. Chronic gout is extremely rare these days, due to the advent of dietary therapy and drugs that lower uric Acid levels. Some degree of kidney dysfunction occurs in nearly ninety percent of subjects with gout as a result of uric Acid deposits, and there is a higher risk of kidney stones.
Causes of Gout
Gout is classified into two major categories: primary and secondary. |

| Uric Acid is more likely to crystallize at lower temperatures, which may explain why roughly 90 percent of gout attacks affect cooler extremities like the big toe.
Acute pain is usually the first symptom. Then the affected joints become inflamed, almost infected-looking— red, swollen, hot, and extremely sensitive to the touch. Repeated attacks of gout over a long period of time can lead to joint damage.
It should be noted that uric Acid is not a fundamentally harmful substance, but a powerful antioxidant, almost as effective as vitamin C, that helps to protect cells from oxidative damage. | | If you have repeated attacks of canker sores, check for nutritional deficiencies.
Q To avoid getting canker sores, it is important to maintain a proper balance of minerals, acidity, and alkalinity in the body. (See Acid /alkali imbalance in Part Two for the Acid and alkaline self-test.) Have a hair analysis done to test for mineral levels. (See hair analysis in Part Three.)
Q Do not take iron supplements unless your doctor prescribes them. Obtain iron from natural food sources.
Q Consult your dentist if you have a mouth sore that does not heal. | | Caution: Do not take this supplement if you are pregnant or nursing, or suffer from panic attacks, diabetes, high blood pressure, or PKU.

It should be noted that uric Acid is not a fundamentally harmful substance, but a powerful antioxidant, almost as effective as vitamin C, that helps to protect cells from oxidative damage. It is only when levels become abnormally elevated that it becomes a problem. Approximately 70 percent of people who suffer from gout produce too much uric Acid, and 30 percent cannot properly eliminate it from their bodies. About 25 percent have a family history of gout. | | Supplemental phenylalanine, as well as products containing aspartame (an artificial sweetener made from phenylalanine and another amino Acid, aspartic acid), should not be taken by pregnant women or by people who suffer from anxiety attacks, diabetes, high blood pressure, phenylketonuria (PKU), or preexisting pigmented melanoma, a type of skin cancer.
Proline
Proline improves skin texture by aiding in the production of collagen and reducing the loss of collagen through the aging process. It also helps in the healing of cartilage and the strengthening of joints, tendons, and heart muscle. | | Gout
Gout is a disease that causes sudden, severe attacks of pain, tenderness, redness, warmth, and swelling in some joints. It usually affects one joint at a time, especially the joint of the big toe. Needle-shaped uric Acid crystals that precipitate out of the blood are deposited in the joint to cause the pain and swelling associated with gout. In people with gout, the body does not produce enough of the digestive enzyme uricase, which oxidizes relatively insoluble uric Acid into a highly soluble compound. |

| The empty seed-pods, often called "monkey's pots," are used to carry around small smoky fires to discourage attacks of black flies, as cups to collect rubber latex from tapped trees, and as drinking cups. The husks of these seedpods have also been used in Brazilian folk medicine to brew into tea to treat stomachaches, and the tree bark is brewed into tea to treat liver ailments.
PLANT CHEMICALS
Brazil nut oil contains mainly palmitic, oleic, and linoleic and alpha linolenic acids and small amounts of myristic and stearic acids and phytosterols. | | Julian Whitaker, MD, reports using cat's claw for its immune-stimulating effects, for cancer, to help prevent strokes and heart attacks, to reduce blood clots, and for diverticulitis and irritable bowel syndrome.
Cat's claw has several groups of plant chemicals that account for much of the plant's actions and uses. First and most studied is a group of oxidole alkaloids that has been documented with immune-stimulant and antileukemic properties. Another group of chemicals called quinovic Acid glycosides have documented anti-inflammatory and antiviral actions. |

| But, he says, use alternative home remedies as well, along with a corrective diet to prevent future attacks.
HIGH-PURINE FOODS: A Must to Avoid
One way to control uric Acid levels is to avoid eating too many foods rich in purine, a protein-related substance that the body turns into uric Acid.
"If you avoid high-purine foods, gout attacks won't be as common," says Dr. Aesoph, "and when they occur, they won't be as severe. | | One caution: This technique is so effective that you may be tempted not to bother with trying to prevent gout attacks. That would be a big mistake, says Dr. Holder, because the disease would still progress, slowly but surely destroying your joints. If you have a gout attack, use this technique. But, he says, use alternative home remedies as well, along with a corrective diet to prevent future attacks.

Taking a tablespoon of apple-cider vinegar each morning is very effective in helping to prevent gout attacks, says Dr. Holder. For severe cases—frequent and very painful attacks—a tablespoon morning and evening may be necessary.
VITAMIN B6 AND MAGNESIUM: For Better-Hydrated Tissues
Vitamin B6 helps distribute water in the body to keep all of its tissues maximally hydrated, which helps prevent uric Acid from turning into crystals. Magnesium aids in the absorption of vitamin B6.
Dr. | | FATTY ACIDS: Reduce Lung Inflammation
The lungs become inflamed during asthma attacks, which makes it harder to breathe. To reduce inflammation naturally, turn to the omega-3's found in fish and flaxseed oil.
"Asthma involves both an acute inflammatory response and a secondary, late-phase inflammatory reaction that can occur up to 24 hours later and lasts for weeks," explains Dr. Firshein. "The late-phase response is now believed to be the cause of chronic asthma and tissue damage, and it can be halted by omega-3 fatty acids. | | People who reduce the amount of fat and sugar in their diets while at the same time increasing fiber intake may be able to stop gallstone attacks because the bile will be less concentrated, making stones less likely to form.
FATTY ACIDS: Keep Stones Small
While cutting down on fat is a must for people with gallstones, increasing your intake of omega-3 fatty acids—the kind of fats found abundantly in fish, flaxseed oil, and olive oil—is recommended.
High-quality oils help improve the solubility of bile, stopping the growth of stones, says Lipski. | | CARNITINE: Boosts the Power of CoQ}0
People with angina who take carnitine (a vitamin-like amino acid) along with coQ10 are much more likely to have reductions in angina pain and frequency of attacks than those who take coQ10 alone, says Michael Janson, M.D., consultant physician at Path to Health in Burlington, Massachusetts.
Alternative practitioners believe that without enough coQ10 and carnitine, the cells of the heart can't make energy from fats. Instead, they burn sugar, or glucose, for energy. |
