For example, the drug company roche created a program for overweight patients who were prescribed Xenical, its weight loss drug. The company asked doctors in Iowa and throughout the country to give their patients starter kits for its Xenicare program. roche asked these patients thirty personal questions, logged the information into its sophisticated database, and then used it to act like a coach or well-wisher to the person. The company sent holiday cards and congratulatory notes as the weeks passed and the patient continued taking the pills. |

As it turned out, roche couldn't even find the small number of social phobia patients needed to enroll in clinical trials of the company's drug, and the clumsy PR campaign was quietly abandoned.
Roche's plain-talking managing director in Australia, Fred Nadjarian, would later claim to have been "taken in" by estimates given to him by his marketing staff of how widespread the condition was supposed to be. "I thought there might be a big market," says Nadjarian, "but when we tried to recruit [for the trials] we just weren't able to. |

The Most Popular Supplement
The Swiss pharmaceutical company Hoffman-La roche was the first to mass-produce vitamin C. Today, many thousands of tons of ascorbic acid are produced synthetically from glucose. The glucose is often derived from corn syrup.
Many consumers believe that vitamin C is beneficial, as about 30 percent of the U.S. adult population takes supplemental vitamin C. Currently, vitamin C is the most widely used vitamin supplement in the world.
Vitamin C is also used extensively to preserve food. |

Leo Sternbach of Hoffman-La roche, Inc., Valium was one of the benzodiazepines, quick-acting antianxiety drugs which are, in effect, minor tranquilizers. Sternbach, who died in 2005 at the age of ninety-seven, was a kind of genius pharmacologist who also invented the sleeping pill Dalmane, the antiseizure medication Klonopin, and Arfonad, which limits bleeding during brain surgery. He held 241 drug patents by the end of his career.15 Far and away his most famous creation, though, was Valium—which he tested on himself only to discover that it made him depressed. |

In the case of Roche's obesity drug Xenical, for example, NICE'S independent review estimated a CQG of £46,000 ($82,800). Translated, this represents a cost to the NHS of £46,000 a year to bring the patient's quality of life up to that of someone who was not obese -which would be cheap if it actually delivered on its promise, but that is not the point. roche, meanwhile, had arrived at a CQG of just £10,000 ($18,000).
Where the Alzheimer's drugs were concerned, the discrepancies were greater. |

A more recent example is the story of Roche's new HIV/AIDS drug, Fuzeon.20 Approved by the FDA in 2003, this drug is an important advance in AIDS treatment. According to a detailed story by the Wall Street Journal reporter Vanessa Fuhrmans, Fuzeon was discovered at Duke University, developed by a local biotechnology company, and only then acquired by roche. Despite its minimal contribution to early research and development, roche charges $20,000 a year for the drug— three times the price of most AIDS drugs. |

Peter Schifferdecker, a doctor working for roche in Switzerland, wrote in a report for the FDA that Accutane "probably caused" depression, that doctors and patients should be warned, and that patients should be monitored for the development of the condition. As reported in USA Today, the company's U.S. marketing team "rewrote" his report because of concerns about its "impact on marketing strategy and product liability," removing the language about how Accutane could cause depression in some patients.
The FDA received an edited report that did not include these concerns about depression. |

In 2000 roche changed the package warning to include depression, psychosis, and suicide as possible side effects. It took the suicide in 2000, of a boy being treated with Accutane who was the son of U.S. Congressman Bart Stupak (D-Michigan), to get the attention of the FDA, which convened an advisory committee to discuss the relationship between Accutane and depression that same year. The committee concluded that "more research" was needed on the topic. |
| In 1986 roche changed the package insert to include information that some patients had reported becoming depressed on Accutane. As reported in USA Today on December 7, 2004 (Kevin McCoy, "Drug Maker Rebuffed Call to
Monitor Users"), however, as of 2004 the company's spokespersons still publicly denied that there was a "causal relationship" between Accutane and depression, meaning that depression spontaneously developed, unrelated to Accutane, but secondary to other causes. |
| Since its approval for the indication of cystic acne by the FDA in 1982, Accutane, manufactured by the Swiss-based company roche, has been prescribed for 2 million patients in the U.S. and more than 8 million patients worldwide.
Extremely effective in treating severe acne, Accutane quickly became very successful, bringing in more than a billion dollars a year in sales, the result, in part, of doctors who also started prescribing it for patients suffering from "treatment-resistant" acne or the "psychological consequences of acne. |

At the same time, roche Laboratories had offered physicians twelve hundred dollars for prescribing the antibiotic Rocephin to twenty patients, according to a letter written on corporate letterhead. And the drugmaker Sandoz was sending checks for a hundred dollars to physicians who agreed to read a two-page article on how its organ transplant drug Sandimmune could treat psoriasis, a skin disease. Sandimmune had not been approved to treat psoriasis. |

However, after planning a study with roche, the acting head of FDA Drug Surveillance concluded in 2002 that such a study could not be blinded because patients would know from the side effects (like skin dryness) what they were on and that therefore the study should not be performed. At that time the FDA knew of 173 cases of suicide from its MedWatch Program. Assuming that 1% of cases get reported, that would mean 17,300 cases of suicide.
My study of the effects of Accutane on brain function using brain imaging was first presented in November 2004. |
| Roche pointed out that depression is so common in the general population that you can't pick out an increased signal over background noise. On the other hand, what is the probability of developing depression solely by chance during a four-month course of treatment? Although the prevalence of depression (meaning that it has occurred at some time in your life) is
16%, the actual incidence (developing new-onset depression) during any given year is only 1.6%.
Therefore, in the four-month period of an isotretinoin trial, about 0.5% of patients would spontaneously develop depression. |
