Since that time, laws regulating ephedra that incorporate the ahpa and NNFA standards for formulation and labeling have been passed in Hawaii, Michigan, Washington, Oklahoma, and Nebraska. In June 1997, the FDA issued proposed regulations on ephedra-containing dietary supplements that would limit the level of total ephedra alkaloids in herbal preparations to no more than 8 mg per dose, with no more than 24 mg per day (FDA, 1997).
Editors' note: It is unclear why ahpa has given a Class 2d safety rating to black tea and not to green tea. Both black and green tea are, in fact, recommended for long-term use for health benefits.) Regulatory Status belgium: Permitted as a traditional herbal diuretic (De Smet et al, 1993). canada: Food (Health Canada, 1997). Substantiated health claims will be permitted with premarket authorization after Natural Health Product (NHP) regulations become final in 2001 or 2002.
The safety rating as assigned by the American Herbal Products Associations Botanical Safety Handbook (BSH) (McGuffin et ai, 1997) is noted, and in some other cases, some European organizations' assessments are also included. The ahpa safety classifications were based on data from more than 30 primary references (including the German Commission E) plus clinical trials, case reports and other forms of evidence.
At the OWH hearing, the Ephedra Education Council (EEC, an industry group) presented the consensus views of a seven-member panel comprised of experts from various medical and scientific disciplines who concluded, based on reviews of both the published literature and the entire public record of more than 1,000 AERs submitted to the FDA, that there was no evidence of an association between ephedra and significant adverse events at the intake levels established by the AHPA's 1994 standards and subsequently adopted as law by several states.

We recommend purchasing herbs sold by reputable companies that have been in the herb business for at least ten years. A number of manufacturers and distributors specializing in herbal products, including herbal combinations for prevention or treatment of certain disorders, are listed under Manufacturer and Distributor Information in the Appendix. If you wish to buy organically grown herbs, look for the "certified organic" label on the product. More than half of U.S. states have certification programs for organic farms and products.

There are organizations now, however, whose job it is to promote and maintain herbal standards, including ahpa (American Herbal Products Association), AHG (American Herbalists Guild) and the Herb Research Foundation. For further guidelines, see the "Shopper's Guide to Quality Herbs" given at the end of this chapter. Herbs will be available as the root, leaf, seeds, flowers, fruit, stems, bark, root bark or any combination of these. For some herbs only one form is used, such as ginger root or chamomile flowers.

The FDA's own expert advisory panel recommended against banning the herb, instead recommending that the FDA institute a policy much like the one ahpa already instituted, but the FDA has not done it—not because it can't, but by choice. WHAT ABOUT DRUG SAFETY? Safety questions about herbs are ironic given about a dozen serious herb reactions over a period of decades, compared to drug reactions at 10 million annually. Yet the drug business continues briskly despite this, unimpeded by the kind of "exposes" to which herbs are subjected.

Although none of these herbs is poisonous enough to rate an ahpa class 3 rating, some are rated as 2a, 2b or 2d. 2a indicates that the plant is for external use only (which would be the case here), 2b that it is not to be used during pregnancy, and 2d that there are other specific use indications, such as in the case of myrrh, a contraindication in the case of excessive uterine bleeding. All in all, if these are the true ingredients, this formula would seem reasonably safe for external use. Whether such treatments do more than just destroy tissue, cancerous and normal both, remains to be seen.

Most importantly, the FDA, in cooperation with the American Herb Products Association (AHPA), appointed a nonpartisan, independent scientific committee (medical experts specializing in gastroenterology and hepatitis research) to review and investigate these cases, and this panel concluded that chaparral be allowed back on market because it found no causal evidence of any harm.

Herbs Against Cancer: History and Controversy

While cat's claw is acclaimed as a potent traditional remedy (including for cancer), it strikes me as odd that there are no mentions of it in any of the standard reference works on herbalism or folk medicine (333). The ahpa states that "ethnobotanical data are scanty." Dr. Fernando Cabieses of the Peruvian Ministry of Health found Uncaria species missing from the oldest and most important books on Peruvian medicinal plants.
The ahpa lists it as a class 4 herb, which means that it has insufficient data available for classification. Scientific reputation is just now beginning to catch up with popular usage. In May, 1994 the World Health Organization sponsored the First International Conference on Una de Gato. At this meeting the vine received official recognition for the first time as a medicinal plant (211). There is some clinical research now underway in Europe using the Krallendorn brand.
Although ahpa considers cloves a class 1 herb, i.e., safe when consumed appropriately, the record is not unmixed: there have been reports of eugenol promoting the growth of tumors (199a). Herbalist Michael Castleman sensibly remarks that cloves are "one of many healing herbs with both pro- and anti-cancer effects. At this point, scientists aren't sure which way the balance tilts. Until they are, anyone with a history of cancer should not use medicinal amounts of cloves." Clark's second herb is a tincture of black walnut rind. This is an old anti-parasite folk medicine.
Also Note: ahpa Ratings are as follows: 1 = generally safe; 2a = for external use only; 2b = not to be used during pregnancy; 2d = other specific use indications; 3 = use with caution; 4 = insufficient data. Great care should be taken to procure fresh herbs, Jones cautioned. The ingredients should be mixed and coarsely bruised. Then they are moistened with dilute alcohol and left to stand for two to three days. They are then put in a steam displacement apparatus and passed through the vapor of three pints of alcohol.
Black walnut is a class 2 herb. ahpa warns that prolonged use is not Eugenia caryophyllata (cloves) advised due to the presence of significant quantities of juglone, a known mutagen in animals. Carcinogenic effects have been associated with the external use of Juglans regia in humans. So like cloves, the record is decidedly mixed. Finally, there is wormwood (Artemisia absinthium), so named because of its historic use as a vermifuge, i.e., an agent to kill worms. I am unaware of any studies showing that it was ever traditionally used against cancer. Dr.

Although these projections are reassuring, ahpa cautions that the information is insufficient to lift goldenseal from the "at risk" list. A second survey for 1999 (to be released in 2000) may provide additional insight, hopefully with cause for more optimism.3 Several experimental cultivation projects with goldenseal have been ongoing for many years.
